Community Health Assessment and Action Plan (CHAAP)

The CHAAP aims to develop a comprehensive and community-driven strategy to enhance health outcomes and resilience in the Town. Clarion will lead this effort by first collecting and analyzing local health data, reviewing existing Town policies, and conducting structured interviews with key stakeholders to identify health infrastructure gaps, current assets, and opportunities for improvement. This work will be aligned with Routt County’s recent health assessment efforts and will involve collaboration with Town staff to ensure the most relevant and up-to-date information is incorporated.

Following data collection, Clarion will facilitate community engagement through an in-person open house and an online survey to gather feedback on community health priorities, vision, and resource needs. Based on these inputs, Clarion will draft the CHAAP, outlining actionable strategies, collaboration opportunities, and a long-term vision for health in Hayden. After internal staff review, a Public Review Draft will be shared with the community for feedback, with final revisions made before presenting the CHAAP to Town Council for adoption. This process is designed to build community ownership of the plan and ensure it is rooted in the lived experience and aspirations of Hayden residents.
